Your first visit to Yale Podiatry Group establishes a vital foundation for our relationship with you. To understand what to expect for your first visit to our practice, please read through this page.
Please aim to arrive 10-15 minutes before your appointment time to allow time to find the office and register.
During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. It is wise to bring along some examples of regular footwear so the podiatrist can assess the cause of the problem as well as treating the presenting symptoms.
What to Bring
- Your current insurance card. Please bring the physical card with you, it will need to be scanned in a card reader. More information regarding insurance and billing can be found here.
- Co-payments are due at time of service. We accept cash,checks, Visa, MasterCard, American Express, and Discover. Checks can be made payable to “Yale Podiatry Group”.
- A photo ID.
- A list of current medications is recommended, especially if you suffer from several medical conditions. Do not omit vitamins and over-the-counter medications.
- Footwear you wear most often. (Excluding flip-flops)
We know you have many choices when choosing a podiatrist in Ansonia & Shelton, CT. We have made requesting an appointment a simple process via our Appointment Request page. For any reason, you cannot keep a scheduled appointment or will be delayed, please call as soon as possible. There is a $25 fee for failing to cancel 24 hours in advance, or for missing your appointment (No-Show). Please note there is a separate fee schedule for cancelled procedures & surgeries. This policy will be discussed with you prior to scheduling either.
